In order to receive the 100% Club Secretary’s Award, each club secretary who has qualified must have the club president fill out this form and have it approved by the proper district officers. The completed form must be forwarded (faxed, emailed, or mailed) to the District Cabinet Secretary no later than July 15 following the close of the Secretary’s year of service being considered. Do NOT forward this application to LCI as the award is not awarded by LCI, but rather awarded by the District.
